How to Go to PA School for Free: The NHSC and Me

The median PA graduating today owes $137,500. I graduated owing nothing — and got a paycheck while I did it. Here's exactly how the National Health Service Corps works, what it pays in 2026, what it asked of me in return, and the one thing about it I'd want you to know before you apply.


Let’s start with the number that keeps you up at night: $137,500.

That’s the median education debt for a newly board certified PA — not the worst case, just the middle. Nearly one in ten of your future classmates will walk across that stage owing $225,000 or more. (NCCPA, Statistical Profile of Recently Board Certified PAs)

I want to tell you about the years I spent not thinking about that number at all.

I am a National Health Service Corps Scholar and a PA

There used to be two types of us. Now there are three:

  1. Those who do loan repayment — you finish school, you take a job at an approved site, and the NHSC pays down the loans you already have.
  2. Those who are part of the scholarship program — they pay for school before you owe anything. This was me.
  3. Those who get in through Students to Service — a newer lane, open to PA students in their final year, that didn't exist when I applied. If you're a rising second-year reading this, keep scrolling. This one is for you.

I was one of the lucky ones who had the privilege of being part of the scholarship program. I received a monthly stipend while in PA school to help pay for books, medical supplies, housing, food, etc., etc. — and then, as part of my agreement, I was "required" to work in an underserved community for two years.

I put "required" in quotes on purpose.

For me this was a win-win, because I wanted to work in that kind of community anyway. The fact that they were going to pay me to do it was just icing on the proverbial cake.

Here's the part nobody tells you: the obligation was never the price. The obligation was the point.

The National Health Service Corps is far more than just a "way to pay for PA school." It is a gratifying and rewarding experience!

I applied for the NHSC scholarship twice

I want to be clear about this, because I think it's the most useful thing in this whole post.

I did not get it the first time.

As a student at UMDNJ (now Rutgers), we had a three-year program. I applied my first year and didn't even make it to the interview.

I was crushed. I remember thinking the committee had looked at my file and seen exactly what I was afraid they'd see — an entirely average student from an entirely average background who'd already been beaten by organic chemistry once.

The next year I applied again. I told the same story, but I told it better, and I told it with a year of actual clinical experience behind it.

That time I got in.

This is further proof that "if at first you don't succeed, try, try again" should be a law. If you got a no this cycle, you did not get a verdict. You got a draft.

The site match

I was matched with my NHSC site in 2004.

I won't lie — the search for a practice site was nerve-wracking. The NHSC scores qualifying practice sites with a ranking based on medical need, and in the months before graduation you are essentially shopping for the next two years of your life on a government website.

In 2004, the need was so great everywhere that they eliminated the requirement that scholars work only in the areas with the highest need scores. So my options opened up a bit, and this may be one reason I am not performing rectal exams at a high-security prison in Nebraska.

I landed in Greenfield, California — a farmworking town in the Salinas Valley, mostly Spanish-speaking, mostly agricultural, an hour from anywhere you've heard of.

It was the best clinical education of my life.

I saw pathology in Greenfield that my classmates at big academic centers read about in a textbook and never touched. Advanced, untreated, walked-in-the-door-at-year-ten pathology. I learned to work without a specialist down the hall. I learned to do a lot with a little. I learned that the hardest part of medicine is almost never the medicine.

Four years in, we moved away from Greenfield, and rather than leave the clinic I chose to commute. With some creative scheduling and a brand-new passion for audiobooks, I solved the commute "problem" — and honestly, I came to love that hour in the car.

Twenty-plus years later, I'm still doing the work

Here's the thing I could not have predicted at 27 years old, signing a two-year commitment I was mildly nervous about:

I never left.

Not that exact clinic, forever — but the work. Today I practice family medicine at the Santa Cruz County Health Department's Emeline Clinic and with the Homeless Persons Health Project, taking care of patients who have Medi-Cal, or nothing at all, or who are living in a tent behind a Safeway.

Same job. Different zip code. Twenty-plus years on.

Turns out I'm not unusual. HRSA tracks what happens to Corps clinicians after their obligation ends, and about 81% of the clinicians who finished their service in FY2022 were still working in underserved areas two years later. Of the 31,029 alumni who completed service between FY2012 and FY2023, 87% are still in a shortage area or still in the community where they served. (HRSA / NHSC Reports to Congress)

Read that again, because it's the real finding.

The NHSC is sold as a debt program. It functions as a recruiting program. They don't have to trap you. They just have to get you in the door and let the patients do the rest.

I finished paying my last college loan six years after PA school graduation. I'm proud of that. I was still saddled with about $20K in undergraduate debt plus my first year of PA school before the scholarship kicked in — the NHSC was not a magic wand, it was a very large head start.

The NHSC, the stipend, and the fact that they paid my tuition freed me from the clutches of student debt. Because of that, I've been able to spend my career providing high-quality care in an underserved community instead of chasing the highest bidder.

And I'll say the uncomfortable part out loud: I fear that as more students graduate saddled with enormous debt, they will be unable to practice the medicine they actually want to practice. They'll take the higher-paying subspecialty job — not because they love it, but because the loan servicer doesn't care what you love.

And that’s not a hunch — NCCPA measured it. In its 2025 profile of the entire profession, 16.5% of PAs said their level of educational debt influenced which specialty they chose. Of the PAs who said that, 28.8% landed in surgical subspecialties and only 16.3% in family medicine. Roughly one in six of us picked a career around a loan balance. That's not a moral failing. That's math. The NHSC is one of the few programs that changes the math.


FACT: NHSC participation among new PAs has tripled

FACT: In 2024, 5.6% of recently board certified PAs reported being a National Health Service Corps Scholar or a state or federal loan repayment participant — which requires practicing in a Health Professional Shortage Area (HPSA) or Medically Underserved Area (MUA). That's up from 1.9% in 2018 and 1.8% in 2020. (NCCPA Statistical Profiles of Recently Board Certified PAs, 2018–2024)

One honest caveat: NCCPA widened its survey window in 2023 — from PAs certified six months or less to PAs certified one year or less — so the 2023 and 2024 figures are not a perfectly clean comparison with the earlier years. The jump from 1.9% to 3.1% between 2021 and 2022 happened entirely inside the old methodology, so that one is real.

Flat for six years. Then it tripled.

I don't think that's a coincidence. It's what happens when tuition outruns starting salary.

And the Corps itself? Bigger than you think — and PAs are its fastest-growing piece

  • More than 80,000 clinicians have served in the NHSC since it began placing providers in 1972. (HRSA)
  • 2,172 PAs were serving in NHSC primary care roles as of September 30, 2025 — 11.7% of the entire Corps, and roughly 23% of its primary care clinicians.
  • PA participation grew 26% in a single year while the Corps overall grew about 7%. We are the fastest-growing discipline in the National Health Service Corps.
  • In FY2024, NHSC clinicians cared for roughly 18.3 million patients across 73.2 million visits.

Twenty years ago I was a rounding error in that table. Now there are more than two thousand of us. Hold onto your stethoscopes — this is our program now, too.


What the NHSC actually pays in 2026

This is the section that changes every year, so here is where it stands as of July 2026. Verify everything at nhsc.hrsa.gov before you plan around it.

The FY2026 cycle ran March 17 – April 16, 2026 and is closed. HRSA has not announced the FY2027 cycle; historically it opens in early spring.

NHSC Loan Repayment Program (pays after you owe)

Important, and widely gotten wrong: the old HPSA-score tiering — $50,000 for a site scoring 14–25, $30,000 for 0–13 — is gone. In FY2026 the award is the same regardless of your site's HPSA score. If you're reading a blog post that still quotes those numbers, it's stale.

NHSC loan repayment awards are exempt from federal income and employment taxes by statute. State treatment varies — check yours.

Stay past your initial two years and you can apply for continuation contracts worth up to $20,000 per additional year.

If you are entering your final didactic year right now, put a reminder on your calendar for August 2026. This is the single most actionable thing in this post.

One gotcha: PA applicants must graduate by August 31. If you're on a non-standard timeline, confirm before you invest the hours.


Now the part I'm obligated to tell you: the funding cliff

I'd be doing you a disservice if I ended on the good news.

The Consolidated Appropriations Act, 2026 (H.R. 7148), signed February 3, 2026, gave the NHSC $350 million for FY2026 plus $88 million to cover October 1 through December 31, 2026.

That mandatory funding lapses after December 31, 2026.

As of late July 2026, Congress has not passed an extension or a reauthorization. The House Appropriations Committee advanced an FY2027 bill in June 2026 with $133.1 million in discretionary NHSC funding — real money, but it does not solve the mandatory cliff.

What that means for you, practically:

  • If you're mid-obligation, your existing contract is a contract. Awards already made are not the thing at risk.
  • If you're planning to apply in 2027, watch this through the fall. Cycle sizes have moved with appropriations before.
  • Don't build a financial plan on an award you haven't received. That's true every year; it's truer this year.

The Corps has gone over this cliff and come back before — repeatedly, since the ACA first put mandatory money behind it in 2011. I expect it will again. But you deserve the actual state of play, not a brochure.

And while we're being honest: federal borrowing rules for PA students are also in flux right now. Grad PLUS ended July 1, 2026, and a court order in late June 2026 temporarily restored PA students to the professional borrowing tier ($50,000/year, $200,000 aggregate) after the Department of Education had put us in the lower graduate tier. The Department says those designations "may change as litigation proceeds." Confirm your limits with your program's financial aid office before you sign anything. Frequently asked questions about the NHSC for PAs

Can PAs actually get the NHSC scholarship, or is it just for physicians?

PAs are fully eligible — for all three programs. The NHSC Scholarship is open to students in medicine (MD/DO), dentistry, nurse midwifery, nurse practitioner, and physician assistant programs. There is no smaller "PA tier." We compete in the same pool for the same award.

What's the difference between the NHSC Scholarship, Loan Repayment, and Students to Service?

Timing, mostly.

  • Scholarship = they pay for school before the debt exists. You apply as a student (any year), get tuition, fees, and a monthly stipend, and owe one year of service per year funded.
  • Students to Service = you apply in your final year, get up to $120,000, and owe three years.
  • Loan Repayment = you're already a practicing PA at an approved site, and they pay down debt you already carry — up to $75,000 for two years in primary care.

Rule of thumb: the earlier you get in, the more they pay and the more service you owe. The Scholarship is the biggest total value and the biggest commitment. Loan Repayment is the most flexible.

How much does the NHSC scholarship actually pay a PA student?

Full tuition, all eligible fees, an annual payment for other reasonable educational costs, and a stipend estimated at $1,648 per month for the 2026–2027 school year. For a two-year PA program at a private school, the total package can easily exceed $150,000.

The catch worth knowing: the stipend is taxable — federal income tax and FICA. Budget for roughly a quarter of it going to withholding. The tuition and fee portions are not taxed.

How long do I have to serve, and what happens if I want to leave early?

Two years minimum for the Scholarship (one year per school year funded, four years maximum). Three years for Students to Service. Two years for Loan Repayment.

Breaking a Scholarship contract is expensive — you become liable for the funds you received plus damages, and the government can pursue administrative offset and refer the debt to the Department of Justice. This is not a "pay it back with interest and walk away" arrangement.

Do not sign an NHSC scholarship contract as a financial hedge. Sign it because you want the work.

Do I get to choose where I work?

More than people assume, less than you'd like.

You select from a searchable list of NHSC-approved sites — community health centers, FQHCs, rural health clinics, correctional facilities, tribal and Indian Health Service sites, school-based clinics, mobile units. Loan Repayment applicants already have a job at an approved site. Scholars go through a site-selection process in their final year, and Scholars can be subject to a minimum HPSA score set annually.

I got Greenfield, California — Salinas Valley farm country, an hour from the ocean, and one of the best clinical training grounds I could have asked for. That was partly luck and partly a 2004 policy that opened up the score requirements.

Go visit before you rank it. Talk to the PA who's there now. Ask what the panel size is.

Is NHSC money taxable?

Depends which pot.

  • Loan repayment awards (LRP and S2S): exempt from federal income and employment taxes by statute. State treatment varies.
  • Scholarship tuition, fees, and OREC: not federally taxed.
  • Scholarship monthly stipend: taxable — federal income tax and FICA.

What are my odds? Is it worth applying?

Nobody publishes a clean acceptance rate, so be skeptical of anyone who quotes you one. What I can tell you is that the applicant pool is national and the awards are finite.

Three things that actually move the needle:

  1. Apply more than once. I got in on attempt number two. This is genuinely common.
  2. Have a real, specific, verifiable connection to underserved care — not a sentence about wanting to help people. Which community, and what did you do there?
  3. Look for the lanes with better odds. FY2026 set aside $16.9 million in loan repayment funds specifically for clinicians at Indian Health Service facilities, tribally operated programs, and urban Indian health programs. Almost nobody writes about that.

And if you speak Spanish at a documented level 3 or higher, that's up to $5,000 more on a loan repayment award. Start the proficiency documentation early — it isn't self-attested.

Can I do a residency or fellowship as an NHSC scholar?

You can, but the NHSC does not provide financial support during postgraduate training, and your service clock generally doesn't run during it. If a PA residency is part of your plan, map the timeline with HRSA before you commit.

Can I combine NHSC with PSLF?

This is the question I get most, and it deserves a longer answer than a blog FAQ.

The short version: NHSC service at a nonprofit or public site frequently counts as qualifying employment for Public Service Loan Forgiveness at the same time. Many PAs have run both clocks concurrently. But NHSC dollars pay down the balance, which changes the PSLF calculus, and program rules on both sides have moved recently.

Run your specific numbers with your loan servicer and your school's financial aid office. Don't take a stranger's word for it — including mine.

Is the NHSC actually going to exist when I graduate?

Honest answer: the program's mandatory funding runs out December 31, 2026, and Congress hasn't extended it yet.

The Corps has hit this deadline repeatedly since 2011 and has been extended every time, usually late and usually attached to a larger package. I would not bet against it existing. I would also not sign a lease based on an award letter you don't have.

Watch it. Apply anyway.

I didn't get it. Now what?

You apply again — and in the meantime you go find the other money, because there's a lot of it and most of it goes unclaimed every year. State loan repayment programs, the IHS Loan Repayment Program, Nurse Corps-adjacent state funds, hospital and FQHC sign-on repayment, and hundreds of PA-specific scholarships.

The bottom line

The NHSC did not just pay for my education. It picked my career for me, and it picked right.

I signed a two-year contract at 30 because I needed the money and believed in the mission, in roughly that order. Twenty-plus years later I'm still taking care of people who don't have anywhere else to go, and I have never once wished I'd taken the higher-paying job.

If you are on the fence about the NHSC, I'd suggest you get off the fence and get with the program 🙂

But go in with your eyes open. This is not free money. It's an exchange — real dollars for real years of your life in a place that needs you. If that exchange sounds like a burden, don't sign. If it sounds like the whole reason you wanted to be a PA in the first place, then friend, they are literally offering to pay you to become who you already wanted to be.

Your next step, today: open the NHSC site search and look up the approved sites within an hour of where you live. Not to apply. Just to look. I want you to see how many there are, and how close.

    Hi,
    You mentioned that you received this scholarship during your second year of PA school. Did they repay you for the first year too, or did you pay for the first year yourself?

      Hi Molly! This is a wonderful question. I had to pay for the first year of my 3-year program myself. In fact, this may be why the NHSC did not accept my application the first year – because I was in a three-year program. I could have applied again for loan repayment at my NHSC site after I completed my first two years and had the rest of my loans paid off but they were not offering that as an option at the time.

    I just applied for the NHSC SP 2020, I start PA school July 2020. I hope to be one of the fortunate one’s like you to receive this scholarship. I understand that as a recipient of this scholarship, you will have to give two years of service following PA school graduation. My question: Is there a some base pay or salary while working in the underserved community for the two years?

      Hi Victor, your contract should be handled the same as every other employee at the institution. In other words, they should not pay you less than their starting salary for a non-NHSC scholar with the same years of experience. This will, of course, vary by institution.

    I am 62 with a long history of health service jobs: Acute care CNA, phlebotomy, endoscopy and special procedures technician. I want to retire early and attend PA school and serve where I would be most needed, with poor, elderly, and the Rez. Is there a program u recommend? Oh , and I’m a Woman of Color on a low fixed income. I can move anywhere that is affordable.

    Thank you for publishing this article. I’ve heard that after doing this and working in an underserved area, at your next job, your offered salary would be less than your colleagues (PAs). Have you found this to be true for yourself or for other PAs?

      Hi PQ, absolutely not. In fact, being an NHSC scholar has always worked in my favor. Contract negotiations will be strengthened by your knowledge and experience and having worked at an FQHC where I have been exposed to many different types of challenging situations has helped me to shine at interviews. Now, jobs in these areas, especially in family practice tend to pay less than you may find in private offices, large HMOs or in medical specialty clinics. But potential employers will never inquire about your prior salary and you should negotiate based on the going rate and your years of experience. I worked at my NHSC site for 10 years, transitioned into specialty practice for three then returned back to public health at a different FQHC because I find the work aligns with my passion and purpose. I did take a cut in pay when I went back to work in public health. But that was a conscious decision. Could I have been paid a lot more to work elsewhere? Yes, but as life progresses priorities change and you will have to balance pay against other factors… like the type of healthcare you wish to provide, population you wish to help, and the impact you hope to make in your community and those you have been tasked to serve.

    Hi,
    I am interested in becoming a PA. I am a current sophomore attending North Carolina State University on a full-ride. I really appreciate all your feedback and input here. I think that the PA route is more fitting to my personality and would bring me more joy than an MD route. I have not started on my 1,000 hours of patient contact and I am not really sure how to do so. I also have not shadowed any PA’s but would very much like to. How should I go about finding someone to shadow and fulfill my clinical requirements? Also, is it too late for me to be considering this route? My science GPA is 3.49 and I am nervous that I am running out of time. What do you think I should do? Also, do you need to have taken your courses at the same university? I am planning on studying abroad and completing some of my requirements abroad, will this make my application weaker?

      Hi Loujain, all great questions! You are not too late at all to make the decision to pursue the PA profession. The first thing to do is to set up an appointment with an academic advisor and discuss your goals and make an academic plan that will allow you to fulfill the necessary PA school prerequisites along with your major requirements. You can adjust your major accordingly if necessary. Everyone goes about fulfilling patient care requirements differently. I started my sophomore year by finding a student position at my college health center and volunteering at the University Hospital. This can be a great place to find student health work/study and volunteer positions. I just walked in and started asking about opportunities. I see you have a campus health department as well: https://healthypack.dasa.ncsu.edu/

      If you do not have a University Hospital, I would recommend calling and going in person to hospitals in your area and offer your services. You can go to clinics as well, offer to work for free to get your foot in the door. Consider blood centers as well, as they often have volunteer opportunities. Once you have your foot in the door this can be the best path to finding shadowing opportunities. You will start to work alongside PAs, NPSs, MDs, nurses, etc. etc. and they will get to know you and trust you and you can demonstrate your enthusiasm. You will see that opportunities will begin to present themselves.

      It’s perfectly fine to take classes at CC and overseas universities, again it’s good to speak with your academic advisor if you plan to use these credits to fulfill requirements. Summers are another prime opportunity to find work and volunteer opportunities.

      There are millions of options out there, and the best way to find them is to go out and hit the road. Internet searches, phone calls, and emails are a good start but never work as well as showing up in person.

      Hi Sadia, congratulations!! To answer your questions:

      1) Beyond the 2-year commitment, do you still have to continue working in Primary Care?
      Nope, you can work in any specialty you want. That being said I think the latest statistic reports that 76% of PAs continue to work in the same specialty across their career, but there is nothing that says you have too. I worked in family practice for 10 years, did pain specialty practice for three, then returned back to primary care where I will likely stay for the foreseeable future. But who knows, and this is a great part about being a PA!

      2) Lastly, I noticed the “The stipend payment portion of NHSC scholarship awards are subject to federal income tax”. Can you please comment on this?
      This just means you have to claim it as income and pay income tax on it when you file your taxes.

    Is there anything you did different the second time around applying for the scholarship that made the difference that you know of? Did you change up the application a lot? If so, some info on the topic might help me avoid pitfalls in my first application which i am currently working on! Thanks in advance!
